Description
Mental Health Services are the provision of outpatient psychological and psychiatric screening, assessment, diagnosis, treatment, and counseling services offered to clients living with HIV. Services are based on a treatment plan, conducted in an outpatient group or individual session.
What the Mental Health Counselor will need
- MSW or equivalent education
- Clinical license
- Two years of related experience
What the Mental Health Counselor will do
- Assessment of mental health care needs and documentation of client mental health diagnosis.
- Coordinate services with multidisciplinary care and support services, including case management.
- Document all services provided per grant and organizational requirements.
- Create and implement treatment plan developed collaboratively with the client within 30 days of referral for services. Treatment plan must include: • Clinical mental health diagnosis • Description of the need(s) • Action steps/interventions to address the need(s) • Treatment modality • Timeframes to address the need, including recommended number of sessions • Dated signatures of the client and mental health treatment provider.
