The Lang Def Health and Wellness Coach facilitates evidence-based, best practice, or evidence informed self-management and prevention curricula to community members and to Holy Cross Health colleagues. Provides behavior change strategies to motivate participants to develop and maintain the skills needed to adopt a healthy lifestyle to prevent or manage chronic conditions. Supports participants by providing information, encouraging progress, and working with the participant (or group) to address any challenges or barriers that arise.

General Summary:

Supports participants by providing information, encouraging progress, and working with the participant (or group) to address any challenges or barriers that arise.

The Lang Def Health and Wellness Coach supports the department in fulfilling the following community health goals: decrease avoidable hospital utilization; improve overall health through strategic partnerships, particularly with those addressing social determinants of health, and strengthen integration and linkages by wrapping community health programs and services around defined populations, services of the Holy Cross Health Network and both hospitals.

Supports the Mission of Trinity Health and Holy Cross Hospital.

Minimum licensure/certification required (if applicable):

  • N/A

Minimum Knowledge, Skills, & Abilities :

  • Bachelor's Degree preferred.
  • 3 years relevant experience required.
  • Previous experience with group facilitation required.
  • Knowledge of the principles of behavior change, adult learning, group facilitation and of basic health, nutrition, and fitness principles.
  • Ability to build relationships with individuals and create community within a group. Capable of guiding behavior change without prescribing personal actions or solutions.
  • Ability to communicate effectively with individuals and in front of a small group and to engage in active listening.
  • Ability to work individually or in a team setting and with a diverse group of people (age, ethnicities, life experiences, etc.).
  • Excellent verbal and written communication skills required. Bilingual English and Spanish required.
  • Ability to work flexible hours, including evenings and weekends.

Working Conditions:

Physical Requirements:  Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects,

Visual Acuity: The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading.

Environmental Conditions: Required to work a flexible schedule for evenings and weekends. The worker is not substantially exposed to adverse environmental conditions Normal office environment.

Reporting Relationship:

Reports to the manager, Disease Management. Under the explicit direction of the manager is expected to work independently in accordance with the strategic plan, policies, procedures, performance expectations, and standards of practice.

Responsible for reporting and identifying all safety hazards, safety violations, and safety-related incidents to the manager and /or safety office or chair of the hospital safety committee. Responsible for strictly adhering to safe work practices, as noted in hospital safety policies and applicable Federal, State, agency and safety-related requirements and regulations.
